What You'll Do

As a Discharge Planner, you will play a key role in supporting patient care and recovery by:

  • Establishing and maintaining a welcoming, therapeutic relationship with clients
  • Collaborating with physicians, nurses, emergency screeners, counselors, and other staff to ensure high-quality patient care
  • Coordinating discharge planning and follow-up care to reduce readmissions
  • Conducting family meetings and gathering collateral information to support treatment and discharge plans
  • Actively participating in daily treatment team meetings to assess client progress and outcomes
  • Leading psycho-educational groups and therapeutic activities as directed
  • Assisting in client safety interventions, including participation in restraints when necessary
  • Supporting program and departmental goals through additional duties as assigned

Who We're Looking For

Minimum Qualifications:

  • Bachelor's degree in social work, Counseling, Psychology, or a related field
  • New graduates are encouraged to apply
  • Prior experience in discharge planning or case management preferred (especially in behavioral healthcare settings)
